Heart Failure

Heart failure, also known as congestive heart failure or CHF, is a chronic (long-term) condition caused by the inability of the heart to pump as much blood as the body needs. It is the most common cause of hospitalization for those over 65 years of age. Initial symptoms often include shortness of breath and a build-up of fluid in the feet and legs. The effects of heart failure and future hospitalization can be managed and minimized with a healthy lifestyle and proper medication. The indicators below offer insight into how well we manage the care of patients hospitalized for heart failure.
