Birth Trauma
Birth trauma is the unintentional harm to a newborn that occurs during birth and requires medical intervention. Birth trauma is reported as a rate by calcultaing the number of birth traumas per thousand births.
Neonatal Mortality
Neonatal mortality is a death of a live born infant during the first twenty-eight days prior to hospital discharge. This pertains to infants greater than or equal to twenty-four weeks of gestational age without lethal congenital malformations or genetic abnormalities who are born in the hospital.

Falls

Falls w/ serious injury
A patient fall that requires medical intervention such as sutures or surgery. A patient fall is counted when a patient falls from bed, falls from a slippery floor, or after fainting for example. Assisting a patient to the floor (cases when a staff member attempts to minimize the im[act of the fall) is considered a fall. Falls with serious injury is calculated per 1000 patient days.

Facility acquired pressure ulcers
Pressure ulcer (hospital-acquired) is an obserable pressure related alternation of intact skin which was not identified and documented with the first twenty-four hours after admission. Pressure ulcer (hospital-acquired) incidence rate is the number of patients with a hospital-acquired pressure ulcer per 1000 days.
